Family Medicine Physician

Martha Wafer-Williams, MD.

Dr. Wafer is a board‑certified family medicine physician who has been proudly serving the people of Louisiana since 2009. She earned her medical degree from the University of Texas Medical at Galveston and completed her residency at the Louisiana State University School of Medicine in Shreveport.

Dr. Wafer provides comprehensive medical services focused on prevention, wellness, and recovery. Her approach combines clinical expertise with genuine compassion for every patient she serves.

Originally from Dallas, Texas, Dr. Wafer is an active member of her local church.

Internal Medicine Physician

Harlan Weiss, MD.

Dr. Weiss is an experienced internal medicine physician with over 30 years of in‑depth clinical expertise. He earned his medical degree from Creighton University School of Medicine and completed his residency at Highland Hospital in California. After retiring from the VA hospital system, he now brings his thorough, patient‑focused approach to Synergy Healthcare.

Known for his detailed evaluations, Dr. Weiss spends time with each patient to ensure every concern is fully addressed. He is a devoted father and grandfather, committed to both family and lifelong learning.

Physician Assistant-Certified

Kristy Kehoe, PA-C.

Mrs. Kehoe is a compassionate and experienced Physician Assistant who graduated from the Louisiana State University School of Medicine in Shreveport. She has been practicing since 2009, providing attentive, high‑quality care to every patient she serves.

She is happily married and a proud dog‑mom to two fur babies. Mrs. Kehoe especially enjoys caring for veteran patients, but she’s equally dedicated to supporting all members of our community as part of the Synergy care team.

A lifelong Disney fan, she brings warmth, positivity, and a touch of magic to every patient interaction.

Physician Assistant-Certified

Stefanie Harden, PA-C.

Mrs. Harden is a dedicated Physician Assistant who graduated from the University of Tennessee and began practicing in 2015.

As Synergy’s full‑time telehealth provider, she offers convenient, high‑quality care from her home office in Tennessee. Licensed in Louisiana, she ensures patients receive the same attentive, personalized care virtually as they would in person.

She is happily married, a proud mom to three little girls, and an enthusiastic soccer coach who loves spending time outdoors.
